I wasn't able to find a pattern for it, but it occurred with me or players from my sessions a total of 3 times. Two of them were with laptops and the other one was the hiveswap cd. We'd just drop the card that had the item and it would simply disappear, although in the hiveswap cd case, it just fell trough the floor of a higher floor of the house into the grass below. It was the only one of the three times it happened that we were able to recover it.

I think this is an issue we already dealt with (the Sburb CD), so it might be related to 3D models...
