while attempting one of the dungeons in the medium, i was swarmed by a crowd of relatively small enemies and pushed into a corner, from which i fell through the floor/wall out of the world. it then gave me the death message and when i said "take me back" it took me back to the beginning of the dungeon, where i then died (presumably from fall damage) and dropped all my stuff on the ground and it gave me the death message again. when i say "take me back" again, it spawns me back in the same spot, so i can collect my stuff and continue. this repeated several times.
